IPC Health Ltd. is recruiting for a Part Time Aged Care Assessor to support the SAS program across Hobsons Bay, Brimbank, Melton and Wyndham. The role focuses on holistic assessments, team-oriented care planning and helping older clients live independently at home.

You will manage a caseload, use My Aged Care tools and maintain accurate client data, with travel across multiple LGAs and an option to work from home where feasible. A fixed term until 30 June 2027 applies.
