NSW Health is seeking a Dietitian for the Transitional Aged Care Program in Wagga Wagga. The role focuses on nutrition assessments, care planning, and interventions to support older adults transitioning from hospital to home within the Wagga Wagga region.

You will work within a multidisciplinary team to deliver person-centred care, home visits, and ongoing outcome reviews, supporting quality of life and independence for clients.
