Community Mental Health Clinician/Case Manager (Enhanced Clinical Stream)
Monday-Friday
Hotham Street Community Mental Health - Preston
Occupational Therapist 3, Social Worker 3, Psychologist 2 or Mental Health Nurse 3
About the Role
An exciting opportunity exists for an enthusiastic Occupational Therapist, Social Worker, Psychologist or Mental Health Nurse to join the Enhanced Clinical Stream at Hotham Street Community Mental Health in Preston.
Within Northern Health Mental Health Division, the Integrated Community Team provides intensive, recovery-oriented mental health services to adults experiencing serious mental illness requiring assessment, treatment, care coordination and support. Services are delivered within a bio-psychosocial framework in partnership with consumers, carers, families and broader support services.
The Enhanced Clinical Stream provides intensive clinical treatment and outreach to consumers with complex and persistent mental health needs. Clinicians work within a multidisciplinary, team-based model to provide assertive engagement, evidence-based interventions, care coordination and recovery-focused treatment to a small, capped caseload of consumers. While consumers are supported by the broader team, each consumer is allocated a primary clinician to coordinate their care.
The stream is designed to support consumers who require a higher intensity of treatment and rehabilitation to consolidate gains in recovery, improve functioning and wellbeing, and reduce the need for more intensive service responses over time.
At Hotham Street you will find an enthusiastic, friendly and professional multidisciplinary team. We work collaboratively,
respect the expertise of each discipline and support autonomous practice while fostering strong team-based care. Professional development, reflective practice and clinical supervision are encouraged and well supported.
Arrange a site visit and chat with us over a coffee. Once you have seen our service and spoken to our team members, you will discover a rewarding opportunity to make a meaningful difference in the lives of consumers and their families.
