THE ROLE -
Provide intensive case management and transitional support to individuals and families exiting mental health facilities and experiencing or at risk of homelessness.
Build trusted relationships through assertive outreach, empowering clients to achieve their goals and maintain independent living.
Connect clients with housing, health, community and support services to improve wellbeing and long-term outcomes.
Collaborate with community agencies and stakeholders to deliver integrated, client-centred support across Gippsland.

THE IDEAL CANDIDATE - what you will bring.
A genuine passion for supporting people experiencing mental health challenges, homelessness, or housing instability, with a commitment to person-centred and recovery-oriented practice.
Solid relationship-building and communication skills, with the ability to engage clients, advocate effectively, and work collaboratively with a range of stakeholders.
Experience in case management, support coordination, community services, mental health, housing, or a related human services field.
Resilience, initiative and sound judgement, with the ability to manage a diverse caseload, work independently, and adapt to changing client needs.
Valid Victorian Driver's Licence |Working with Children Check |Police Check
Minimum requirement of Diploma of Community Services or other relevant or appropriate tertiary qualifications and/or experience relevant to this position.
