About the Role
We are seeking a dedicated and compassionate Community Worker to join our growing team. The successful applicant will provide person-centred support to people with disability across home and community settings, promoting independence, social inclusion, and achievement of individual goals. This role combines direct support work with community engagement and service access activities.

Key Responsibilities
- Provide person-centred support to people with disability.
- Assist with personal care, meal preparation, domestic assistance, mobility support, and community access.
- Support participants to attend appointments, educational programs, employment opportunities, and social activities.
- Promote independence, confidence, skill development, and community inclusion.
- Support participants to access health, housing, employment, educational, and community resources.
- Build positive relationships with participants, families, carers, allied health professionals, and other stakeholders.
- Maintain accurate case notes, support records, and incident reports.
- Assist participants to achieve goals outlined in their NDIS plans.
- Follow organisational policies, NDIS Practice Standards, and the NDIS Code of Conduct.



Essential Requirements
- Certificate III or IV in Individual Support, Disability, Community Services, Mental Health, Aged Care, or a related qualification.
- Experience supporting people with disability and/or complex support needs.
- Strong communication and interpersonal skills.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR Certificate.
- Current Australian Driver Licence.
- Access to a reliable and insured motor vehicle. Desirable
- Experience working within the NDIS sector.
- Knowledge of NDIS Practice Standards and Quality Indicators.
- Experience supporting community participation and skill-building activities.
