Best Care Services is seeking a motivated and compassionate Physiotherapist to join our growing Aged Care and NDIS team in Fairfield, Sydney.

This is an opportunity to deliver meaningful, person-centred physiotherapy services to people with disability and older Australians, supporting them to improve mobility, maintain independence and achieve their individual goals.

Working across our onsite clinic and clients' homes, you will deliver evidence-based assessments and interventions tailored to each person's needs, abilities and living environment.

We are looking for a clinician who values independence, dignity and quality of life and enjoys working collaboratively with clients, families and multidisciplinary care teams.

Key responsibilities

- Conduct physiotherapy assessments and develop individualised treatment and management plans.
- Deliver evidence-based interventions to improve mobility, strength, balance, functional capacity and physical wellbeing.
- Support falls prevention, mobility assessments, transfers, equipment recommendations and protected movement strategies.
- Provide physiotherapy services to NDIS participants and older people receiving in-home aged care services.
- Conduct home visits to assess clients' functional abilities, home environments and equipment needs.
- Deliver clinic-based consultations and therapy sessions at our Fairfield location.
- Prepare clinical reports, progress notes, assessment outcomes and recommendations.
- Monitor client progress and review treatment plans in line with individual goals and changing needs.
- Collaborate with support workers, nurses, occupational therapists, GPs, families and other healthcare professionals as appropriate.
- Provide education and practical guidance to clients, families and support workers to promote safe mobility and independence.
- Maintain accurate clinical documentation and comply with professional, privacy,



safeguarding and organisational requirements.

Essential requirements

- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Physiotherapist.
- Relevant tertiary qualification in Physiotherapy.
- Experience or demonstrated capability in assessment, treatment planning and evidence-based clinical practice.
- Strong communication, interpersonal and documentation skills.
- Ability to work independently while collaborating effectively with a multidisciplinary team.
- Current driver's licence and willingness to travel for community home visits.
- Appropriate working rights in Australia.
- Willingness to obtain and maintain relevant worker screening and clearances required for the role.

Desirable experience

- Experience working with older people in community, home care or aged care settings.
- Experience supporting NDIS participants with physical disabilities, neurological conditions, mobility limitations or complex support needs.
- Knowledge of falls prevention, functional assessments, mobility aids and assistive equipment.
- Experience conducting home-based assessments and providing practical recommendations to improve safety and independence.
- Familiarity with NDIS documentation, reporting and funding arrangements, and the Australian Government's Support at Home program.

Pay: $79,000.00 – $130,000.00 per year
