Circle Home Care is looking for an experienced Care Manager (Registered Nurse) to join our growing Melbourne-based team.

For us, this role is about relationships first, followed by action.

We want someone who takes the time to know their clients, understands what matters to them and becomes someone they can trust and rely on. Just as importantly, you need to be proactive. When something needs attention, you follow it through.

You’ll manage a portfolio of Support at Home clients, assessing their wellbeing and care needs, developing personalised care plans, coordinating services, managing budgets and responding when circumstances change.

What you’ll be doing

- Build genuine, trusted relationships with clients and their support networks.
- Assess clients’ health, wellbeing, goals, care needs and risks.
- Develop and maintain practical, person-centred care plans.
- Recognise changes in health or circumstances and take appropriate action.
- Coordinate nursing, care workers, allied health and other services.
- Manage client budgets and service utilisation under Support at Home.
- Respond promptly to clients and follow through on agreed actions.
- Maintain accurate care documentation and manage incidents and risks.
- Work closely with our Directors and broader care team to deliver responsive, high-quality care.

About you

You’ll be an experienced RN and care management professional who genuinely enjoys working with older people.

You’ll need:

- Bachelor of Nursing or equivalent.
- Current unrestricted AHPRA registration as a Registered Nurse.
- Practical Support at Home care management experience.
- Experience independently managing a portfolio of home care clients.




- Experience with assessments, person-centred care plans and client budgets.
- Strong clinical judgement and experience managing risks, incidents and changes in condition.
- Excellent communication and relationship-building skills.
- Strong organisation, follow-through and documentation skills.
- Current driver's licence and ability to visit clients across Melbourne.
- Required checks and current First Aid/CPR.
