Client Centred Home Care (CCHC) provides community nursing and support services across South East Queensland & Northern NSW. We take a community-of-care approach – clients are supported by consistent, familiar teams rather than roaming agency staff.

We're looking for a Registered Nurse to join our team and provide clinical oversight, support and direct clinical care within our disability homes and throughout the community. Gold Coast, Full Time.

About the Role

You'll provide clinical oversight for a diverse client group, including many people living with disability or complex neurological conditions such as spinal cord injury, MND, MS, Huntington's, cerebral palsy, stroke and dementia. You'll also mentor the in-home RCO team leaders and support teams working with those clients.

The role combines office-based care management with community-based nursing and work across the Gold Coast region.

Responsibilities include:

- Client assessments and care planning
- Direct nursing care — wound management, catheter care, insulin and medication administration, dysphagia support
- Comprehensive health, wound, diabetes and continence assessments and reporting
- Training and mentoring support workers, RCO Team Leaders and community nurses

What You'll Bring

- Diploma or Degree in Nursing with full, unrestricted AHPRA registration
- Experience managing a caseload of clients
- Current CPR and First Aid certificate
- Ability to assess, plan and deliver client-centred care within your scope of practice
- Strong communication skills and the ability to work both autonomously and within a team
- Sound time management, organisational and problem-solving skills
- Confidence with IT systems and attention to detail
- Blue Card and NDIS Worker Screening Clearance (or willingness to obtain), Australian National Police Check, driver's licence, and legal working rights in Australia

Experience within the Disability, Aged Care or Community Services sector is highly valued.

Job Types: Full time, Permanent

Pay: $80,000.00 – $95,000.00 per year
