Female Disability Support Worker – Community Access

Location: Wanneroo, WA

About the Role

We are seeking a compassionate, reliable, and experienced Female Disability Support Worker to support a participant in Wanneroo, WA.

The successful candidate will provide community access and daily support while promoting independence, dignity, choice, and social participation. A key requirement of this role is the ability to drive a modified vehicle, including a van or SUV fitted to accommodate a wheelchair, to safely transport the participant to appointments and community activities.

Key Responsibilities

- Provide community access and social participation support.
- Transport the participant using a modified wheelchair-accessible vehicle.
- Assist with appointments, shopping, errands, and recreational activities.
- Support safe wheelchair loading, securing, and transportation procedures.
- Promote participant independence and achievement of personal goals.
- Follow support plans, policies, and workplace safety procedures.
- Complete accurate progress notes and required documentation.
- Maintain effective communication with the participant, family, and Open Sky team.

Essential Requirements





- Previous disability support experience.
- Experience driving a modified wheelchair-accessible vehicle, including vans or SUVs.
- Current NDIS Worker Screening Check.
- National Police Clearance.
- Current First Aid and CPR Certificates.
- Valid Australian Driver's Licence with a good driving record.
- Confidence supporting participants during community outings.
- Good communication and documentation skills.
- Reliable, punctual, and skilled approach.

You are a patient, confident, and safety-focused support worker who is comfortable driving wheelchair-accessible vehicles and supporting participants in the community. You are committed to delivering person-centred support while maintaining the participant's dignity, independence, and wellbeing.
