Would you like to design individual support arrangements with NDIS participants and their families, and build the teams that deliver them?

At Your Best Life Disability and Health Services (YBLDHS), we design support for one person at a time, not around SIL ratios and vacancies. Service for One is our participant-directed model. The participant directs their activities and routines. YBLDHS employs the workers and looks after the funding, records and compliance. Every arrangement is different because no two people are the same. Our first Service for One Coordinator has been a great success, and we’re now looking for a second.

About the role

You’ll work with participants and their families to turn what they want from home and everyday life into a working support arrangement. You will:

- Design each arrangement with the participant, including the needs and preferences profile, quote and budget
- Recruit, match and induct the support team the participant chooses, including people from outside the disability sector who bring skills the participant wants
- Keep arrangements running, with rosters, handovers, practice leadership and regular check-ins with families
- Help prepare each participant’s monthly statement, and make sure records and claims are accurate
- Spot unused plan funding and safeguarding risks, and raise them early

The role is operational and non-clinical, and it reports to the CEO. You’ll get wrap-around support from our HR, finance and quality teams.

About you

This role would suit a former Support Coordinator or someone with similar experience. You’ll be a good listener and a great organiser, and you’ll stay calm under pressure. You’ll also bring:





- NDIS experience (essential), including a sound understanding of participant rights, dignity of risk, supported decision-making and NDIS pricing
- Experience setting up or coordinating individualised supports such as ILO, self-managed or shared-managed arrangements (highly regarded)
- Experience recruiting and matching support workers
- Strong attention to detail with records, budgets and digital systems
- A Certificate IV in Disability, Individual Support, Community Services or a related field, or equivalent experience

You’ll need:

- NDIS Worker Screening Check
- National Police Check
- NDIS Worker Orientation Module
- Current First Aid and CPR
- Current Queensland driver licence and a reliable vehicle
- Working with Children Check (where required)
