Summary:We are seeking a highly organized and compassionate Case Manager to join our team. The Case Manager will be responsible for managing and coordinating the care of clients, ensuring that they receive the appropriate services and resources to meet their needs. The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and have a robust commitment to client advocacy.Responsibilities:Conduct assessments of clients’ needs and develop individualized care plansCoordinate and monitor the delivery of services and resources to clientsAdvocate for clients and ensure that their rights are protectedMaintain accurate and up-to-date client recordsCollaborate with other professionals, such as healthcare providers and social workers,



to ensure comprehensive care for clientsProvide education and support to clients and their familiesParticipate in case conferences and team meetingsMonitor and evaluate the effectiveness of care plans and make adjustments as neededQualifications:Bachelor’s degree in social work, psychology, or a related fieldMinimum of 2 years of experience in case management or a related fieldKnowledge of community resources and servicesExcellent communication and interpersonal skillsAbility to work independently and as part of a teamStrong organizational and time management skillsAbility to maintain confidentiality and handle sensitive informationProficiency in Microsoft Office and electronic medical records systems
