Enrolled Nurse / Support Worker – Remote Care Connector (Holistic Care Solutions) is a clinician-led provider delivering holistic, person-centred care across the community.

We specialise in complex care and support participants to live safely and independently in their homes. We are a small, values-driven team focused on quality care, robust clinical support, and meaningful community-based outcomes.

About the Role

We are seeking Enrolled Nurses and experienced Disability Support Workers for current and upcoming prospects across the Northern Gold Coast and Logan regions.

This is not high-volume agency work, you will:

Work with consistent participants Support complex and high-intensity needs Be supported by experienced clinicians Deliver care in participants’ homes and community settings

Key Responsibilities

Deliver protected, person-centred care to participants with complex needs Assist with personal care, daily living and community access Follow care plans and positive behaviour support strategies
