Access Telehealth is seeking an experienced nurse to assess, prioritise and coordinate care for residents in Traralgon. This in office role is 0.7 FTE with no night or weekend shifts, working alongside GPs and Specialists to ensure timely, high-quality care.
You’ll use digital tools and clinical systems to manage referrals and care plans. You’ll bring robust clinical reasoning, risk recognition and independent decision-making, with 2+ years post-registration experience and aged care exposure.
