Darling Downs Health is seeking a Clinical Nurse Consultant - Frailty Intervention Community Service to support older people with complex health needs across the Toowoomba region. You will assess, plan and intervene to promote secure, person-centred outcomes and coordinate care with families and health specialists.

You will lead multidisciplinary collaboration, drive quality improvements and mentor staff, shaping care across services and systems.
