Croydon Health Services NHS Trust is seeking a Discharge Coordinator to lead complex discharge planning, coordinate the Discharge Navigators and work with ward teams to ensure timely, protected patient transitions to community care. The role requires critical thinking, explicit communication, and a proactive approach to identify and resolve barriers to discharge.

The post is full-time across 7 days with adaptable scheduling.
