Nexia Aged Care is seeking a Care Partner / Care Manager to join our growing Support at Home team in Recent Gisborne, servicing clients across the Western metro/ Macedon Ranges.
This hybrid, part-time role involves home visits, community engagement, and coordinating services to help clients live independently at home.
You will work with our Clinical Lead and Service Coordinator to ensure safe, coordinated care.
