Central Adelaide Local Health Network (CALHN) is seeking a Cardiac Nurse Consultant to lead culturally protected cardiac care for Aboriginal and Torres Strait Islander patients.
You will work with the Heart and Lung Program, Aboriginal Health Unit and community partners to guide clinical pathways from hospital to home.
You will coordinate assessments, care plans and referrals, support discharge planning, rehabilitation and follow-up, and mentor nursing teams while contributing to research and service
