Silverchain Group is seeking a Care Partner to support older Australians in the Central Coast region.
You will assess needs, coordinate services and advocate for secure, independent, in-home care.
This 6-month fixed-term role blends community visits with the flexibility to work from home.
You will work with a supportive team across clinical, scheduling and service delivery to build strong relationships with clients and their families, promoting positive health outcomes.
