About the Role
We are seeking an experienced Care Manager to join a growing Support at Home team in Darwin, NT.
You will manage a portfolio of clients receiving Home Care Package and CHSP funding while ensuring exceptional client outcomes and service delivery
Responsibilities
- Manage a portfolio of HCP, CHSP, aged care clients
- Complete assessments and develop care plans
- Coordinate support services and providers
- Monitor budgets and funding allocations
- Conduct regular reviews and reassessments
- Maintain compliance and documentation
Ideal Candidate
- Previous Case Management or Care management experience
- Experience within Home Care Packages or Community Care
- Strong knowledge of aged care legislation
- Excellent communication and stakeholder engagement skills
- Current Driver's Licence
