The Role

We are seeking an experienced Care Partner, you’ll be the link between clients, families, and support workers. Ensuring care is delivered with empathy, accuracy, and heart. This is a agile role where you’ll combine client engagement, care coordination, and rostering responsibilities, making every day varied and rewarding.

- Key Responsibilities
- Support clients in navigating their Support at Home funding.
- Build strong, trusting relationships with clients, families, and support staff.
- Coordinate services to match each client’s needs, goals, and preferences.
- Create and manage effective rosters for our team of support workers.
