We currently have a full time permanent prospect for an experienced Clinical Care Coordinator for our facility in The Mariner, South West Rocks.
Your main duties will include:



Coordinate the delivery of quality clinical services in line with a resident centred model of care Maintain current knowledge and understanding of Aged Care Funding models Manage and provide efficient, effective and compassionate care planning and assessment Coordinate the availability of medical stock and other health related supplies to ensure sufficient and adequate supplies About you Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) A commitment to work within Calvary's Mission, Vision and Values Demonstrated knowledge and understanding of the legislation governing the Aged Care Commitment to contemporary aged care nursing practice Ability to support a multi-disciplinary team Effective communication skills, oral and written Demonstrated alignment with the philosophy of resident focused care which supports dignity, independence and informed consultative decision making
