ForHealth Group in Dapto is seeking a Registered or Enrolled Nurse to support GPs and patients through chronic condition management and preventative care. You’ll join a friendly team dedicated to improving health outcomes in the local community.

This role offers full-time in office work, collaborative multidisciplinary care, and opportunities to grow in CCM initiatives, health coaching, and patient education.

AHPRA registration and CCM experience are preferred.
