City Health Care Partnership CIC seeks a Health Inequalities Lead to support Smokefree services and reduce health inequalities in Liverpool. You’ll build links with primary care and community partners and drive social prescribing, focusing support where needed most.

You will coordinate stop smoking interventions, help establish community clinics, monitor outcomes, and work with staff and volunteers to deliver high-quality services. This role is full time and based in Liverpool.
