Care Partner (Case Manager)

Loving Hands Care is a Gold Coast home care provider helping older people live safely and independently at home under the Support at Home program. We are small, growing and built around one idea: the right support, from people who know you.
About the role
As a Care Partner you are each client's main contact. You will manage a caseload of about 50 Support at Home clients from your home office and in their homes, planning care, coordinating support workers and services, and making sure every client gets full value from their budget.
What you'll do
- Meet recent clients at home and set up their care plans and service agreements
- Coordinate support workers, allied health and other services with our rostering team
- Track each client's quarterly budget and keep their services on track
- Check in with every client at least monthly and review care plans when needs change
- Refer clients for reassessment, assistive technology, restorative care or end-of-life care
- Keep accurate notes and escalate clinical concerns to our Clinical Manager
What you'll bring
- At least 12 months in case management,
care coordination or community aged care
- Certificate IV in Ageing Support or higher, or a degree in nursing, social work or allied health
- Working knowledge of Support at Home and the Aged Care Quality Standards
- Strong organisation and record-keeping, and confidence managing your own day from home
- Current driver's licence, police check, aged care worker screening and first aid certificate
- Registered or enrolled nurses are especially encouraged to apply
What we offer
Company car included — private use, fuel card, insurance and servicing all covered
- $85,000–$95,000 base plus super, depending on experience (more for registered nurses)
- Work from home with a laptop, phone, $50 a month home office allowance
- Monday to Friday, with flexible start and finish times around client visits
- Paid training and support for further study
