The Social Work Support Coordinator provides skilled social work support coordination for NDIS participants with complex disability, psychosocial, behavioural, and social support needs. The role focuses on person-centred planning, psychosocial assessment, risk-responsive coordination, and the delivery of ethically grounded support coordination that promotes safety, participation, and continuity of care. The Social Work Support Coordinator works with participants, families/carers, and multidisciplinary providers to implement NDIS plans, coordinate funded supports, address barriers to access, and strengthen functional and social outcomes—particularly where complexity requires advanced clinical judgement and systems based coordination.
Pay: From $79,423.00 per year
