Bolton Clarke is seeking a Care Partner based in Forest Hill to manage care delivery for older adults in the community. You will work within the Support at Home program, coordinating care plans and collaborating with clinical teams to meet participants’ needs.

You will liaise with healthcare specialists, review documentation, and support participants and their carers in making informed service decisions. A compassionate, culturally aware approach is essential.
