Silverchain is seeking a Care Partner for the Central Coast region in NSW Australia. This full time, 6-month fixed-term role combines community-based client visits with the flexibility to work autonomously from home, supporting older Australians receiving government-funded aged care services.

You will assess needs, coordinate services, develop personalised care plans, monitor budgets and advocate for positive outcomes that keep clients secure, independent and connected at home.
