Experienced Female Disability & Mental Health Support WorkerYou will provide personalised support to participants with mental health challenges and disabilities helping them achieve their goals, enhance their quality of life, and maintain their independence.
You will work closely with participants, their families, and a multidisciplinary team to deliver holistic and person-centered care.Key responsibilitiesProviding one-on-one support and assistance to clients with daily living activitiesImplementing personalised care plans to help clients manage their mental health and enhance their quality of lifeMaintaining accurate client records and documentationAdhering to all organisational policies, procedures and legislative requirementsSupport participant in developing coping strategies and life skills to manage their health conditions and live as independently as possibleOffering intergenerational support to people anywhere from 14 to 60About youA Certificate III or IV in Individual Support (Disability, Ageing, Home and Community) or equivalentMinimum of 2 years' experience working with individuals with disabilities / mental healthStrong understanding of mental health disorders,
