Centacare Catholic Family Services is seeking a Case Manager to support families with children from unborn to 18 years, delivering therapeutic case management and in-home support.
You will coordinate assessments and develop tailored plans in partnership with DHS, DCP and other professionals to strengthen parenting capacity and child safety.
The role emphasises child-centred, family-focused practice and culturally secure assessment, planning and intervention to enable safe homes and positive family
