Your opportunity:
The Nurse Navigator Service is a state-widemodel of care designedto provide holistic support to patients with complex health conditions requiring an expert level of healthcare co-ordination. This integrated and proactive approach to health care service delivery will help ensure patients receive the most appropriate service when and where they need it. Nurse Navigators work across system boundaries to actively advocate and improve health literacy, in close partnership with patients and families, other health and allied-health services to improve the patient's healthcare experience and quality of life.
Temporary Full Time (76 hours p.f.) from date of appointment for up to 18 months and Temporary Part Time (38 hours p.f.) from date of appointment up to 31st December **** with the possibility of extension.

Our offer:
You will receive a competitive remuneration package value up to $174,208 p.a., comprising salary between $140,632 - $152,685 p.a. (f/t) or competitive salary rates between $***** - $***** p.h. (p/t), employer contribution to superannuation (up to *****%) and annual leave loading (17.5%) (Nurse Grade 7) (Temporary Full Time, 76 hours p.f., from date of appointment for up to 18 months and Temporary Part Time, 38 hours p.f.,
