Lutheran Services is seeking a compassionate Care Partner to support older Australians living at home in Cabanda - Rosewood, QLD. This part time, permanent role offered 56 hours per fortnight focuses on building trusting relationships and delivering person-centred care within budget and funding constraints.
You will meet clients, develop tailored care plans, and coordinate with families and service providers.
