Proactive Health in Toowoomba is seeking a Social Worker to join Knight Healthcare under the Proactive Health network. You'll manage a diverse caseload across DVA, NDIS, Home Care Packages, workers' compensation and private clients, coordinating with physiotherapists, OTs and dietitians to support holistic care in the community.
