NHS Jobs in Horsham is seeking a Lead Practitioner for the Early Intervention in Psychosis Service. The role involves working in partnership with service users with complex health and social care needs, developing care and recovery plans, and applying NICE-recommended interventions with assertive outreach.
The post holder will triage referrals, assess and plan care, provide specialist advice, and collaborate with carers, families and partner agencies to promote safety and well-being.
