Helping Hand Aged Care in Salisbury, SA, invites a dedicated Care Partner to support older people to live safely and independently at home. You'll work with customers, their supports and internal teams to co-design tailored care plans, coordinate services, and promote wellbeing and independence.
You'll bring Certificate III in Health Services Assistance or equivalent, plus hands-on experience in care or case management, with a robust rights-based, person-centred approach.
