Key Responsibilities
Depending on the organisation and position, responsibilities may include:

- Coordinating the delivery of high-quality Home Care and community support services.
- Managing client referrals, service schedules and ongoing care requirements.
- Conducting client assessments and developing individualised care plans.
- Coordinating services under the Support at Home program and CHSP.
- Liaising with clients, families, support workers and allied health professionals.
- Monitoring client wellbeing, identifying risks and responding to changing care needs.
- Supporting workforce rostering, service delivery and continuity of care.
- Maintaining accurate client records, care documentation and compliance requirements.
- Ensuring services align with aged care legislation and quality standards.
- For clinical positions, providing clinical assessments, care planning, risk management and oversight of complex client needs.

About You
We are interested in hearing from individuals with:

- Experience in Home Care, Community Aged Care, Care Coordination or Clinical Care Management.
- An understanding of Home Care Packages, Support at Home or CHSP.
- Experience coordinating services,



managing client needs or developing care plans.
- Strong communication, organisational and relationship-building skills.
- The ability to manage competing priorities and work collaboratively with multidisciplinary teams.
- A genuine passion for supporting older Australians to remain independent in their own homes.
- For Clinical Care Coordinator positions, current AHPRA registration as a Registered Nurse is highly regarded and may be required depending on the role.
