Adelaide

Harmony Nursing Australia Pty Ltd is a South Australian–owned community nursing and care provider based in Salisbury, in Adelaide’s northern suburbs.

The business was founded by a registered nurse with experience in acute hospital nursing and COVID-19 vaccination clinics.

After supporting a family member through illness during the pandemic, she saw how many people in the community were struggling to get good care at home.

That experience shaped our purpose: helping people stay safe, well and independent in their own homes.

We are looking for a compassionate and dependable Personal Care Assistant to join our team in Salisbury, SA.

This is a rewarding opportunity for a caring, motivated professional to deliver hands-on personal care and practical support that makes a real difference to our clients’ comfort, safety and quality of life.

Working closely with our Registered Nurses, you will support a diverse client group, including older people, veterans, NDIS participants and people recovering from illness or surgery.

Key Responsibilities•Deliver warm, person-centred care to clients in their homes, workplaces and community settings across Salisbury and the wider Adelaide area, helping them stay safe, comfortable and independent.•Support clients returning home after a hospital stay or surgery to complete exercise and mobility programs set by their nurse or physiotherapist, helping them rebuild strength and confidence.•Provide standby or full assistance with showering, along with help with grooming, oral hygiene, toileting, continence care and dressing, always protecting each client’s privacy and dignity.•Monitor each client’s physical, emotional and cognitive wellbeing at every visit.

Promptly escalate changes or warning signs, such as new pain, skin or wound concerns, reduced appetite or increased confusion, to the supervising Registered Nurse.•Provide personal care to eligible veterans under the DVA Community Nursing Program, as well as to private clients, NDIS participants and older people receiving in-home aged care services.•Apply individualised therapeutic and behaviour support strategies for clients living with dementia or responsive behaviours,

as set out in their care plans.

These may include consistent routines, reassurance, gentle redirection and meaningful activities.•Help clients take their prescribed medications at the documented times through prompting and assistance with dose administration aids.

Administer medication where you hold the relevant competency, and record all medication support accurately.•Assist clients to move safely in and around the home, including transfers, repositioning and correct use of walking frames, wheelchairs, shower chairs and hoists, using protected manual handling techniques.•Work with the nursing team, clients and families to develop and review individual care plans, sharing your day-to-day observations so each plan reflects the client’s current needs, goals and preferences.•Assist Registered Nurses with basic care within your scope of practice.

This includes checking that dressings remain clean and intact, providing skin and pressure-area care, taking basic observations where trained, and preparing supplies for wound care and post-surgical visits.•Listen to any concerns or complaints that clients or their families raise about their care, record the details accurately and pass them on promptly to the nursing team or management for resolution.•Prepare nutritious meals suited to each client’s dietary needs and preferences, assist with eating and drinking where needed, and encourage good nutrition and hydration.•Communicate with patience and clarity, using simple language, non-verbal cues or communication aids, so clients with hearing, speech or cognitive difficulties can express their needs and make choices about their care.•Contribute to specialised care assessments by sharing observations about clients’ daily living abilities,

home environment and support needs with the assessing nurse.•Carry out practical domestic support to help clients remain independent at home, such as light housework, laundry, bed-making, basic garden tidying and grocery shopping.•Accompany clients to medical appointments, shopping and social outings, providing transport where required.•Maintain accurate, objective and confidential progress notes, care records and incident reports in line with company documentation and privacy requirements.•Follow infection prevention and control practices, including hand hygiene, safe handling of linen and waste, and correct use of personal protective equipment (PPE).•Identify and report hazards in clients’ homes and follow work health and safety procedures.

Respond calmly to incidents or emergencies, providing first aid and notifying the nurse, emergency services or family as required.•Participate in team meetings, supervision and ongoing training to keep your skills current and help maintain care that meets national quality standards.
