Job Description Disability Support Worker Position Title Disability Support Worker Role Purpose

To provide respectful and person-centred support to a 55-year-old participant with mental health needs, assisting with daily living activities, community participation, and overall wellbeing while promoting independence and choice.

Key Duties

- Support the participant with daily living activities such as meal preparation and light household tasks
- Provide emotional support and encouragement in a calm and respectful manner
- Assist with community access, appointments, shopping, and social activities
- Support the participant to build confidence and independence
- Follow care plans, risk assessments, and support strategies
- Monitor wellbeing and report any concerns to the coordinator or manager
- Complete progress notes and timesheets accurately

Skills & Qualities

- Patient, respectful, and reliable
- Valuable communication skills
- Understanding of mental health support
- Ability to work independently and follow instructions
