Sussex Community NHS Foundation Trust Frailty Practitioner role within Integrated Neighbourhood Teams across Sussex offers advanced clinical care to people living with frailty. You will assess, plan and coordinate personalised management with GPs, community nurses, therapists and social care.

You will manage a defined caseload, identify deterioration early and support protected discharge while contributing to service development and education.
