Indiana Skilled Management Group, Inc. is seeking a Case Manager for a full-time, hybrid role serving Lake County, Indiana. The position combines remote work with in-person community meetings several times per week.

You’ll assess needs, create individualized support plans, help clients access services, and coordinate with providers. Applicants must reside within Lake County and be willing to travel within the county and surrounding areas as needed.
