Carrington is seeking a Care Partner to guide older clients through their care journey within the SaH and CHSP programs. You will deliver high-quality case management, coordinate services, and help clients stay independent at home across the Macarthur region.
This full-time role runs 8:00am–4:00pm, Monday to Friday, with in office Grasmere office visits and client home visits. You will join a supportive team in a not-for-profit organisation committed to best practice and compassionate care.
