Silverchain is seeking experienced Home Care Workers to join our Transitional Aged Care Program (TACP) in the Manly area, NSW. You will support older clients transitioning from hospital to home, delivering personal care, mobility assistance and transport coordination in a short-term rehabilitation setting.

Requirements include a Certificate III in Aged Care or equivalent, current driver’s licence, reliable vehicle, and 6 months of relevant experience.
