Jubilee Community Services Inc in Sydney is seeking Community Support Workers to assist frail aged residents and others in the community. This part time role runs Wednesdays and Thursdays, 9 am to 4:30 pm, and involves transport and accompanying clients to appointments and social outings.
You will undertake home assessments, maintain case notes, and support care planning under SCHADS Level 2.1. A Certificate IV in Aged Care, a current driver's license, and background checks are required.
