Open Minds is seeking a compassionate Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.

You will work with a multidisciplinary team, support smooth transitions between services, drive quality improvement and contribute to positive client outcomes through ethical, cooperative practice.
