Mounties Care is seeking a dedicated Care Partner to join our Dandenong office on a part-time basis (3 days per week). You will coordinate care plans, manage client budgets and assist clients to access My Aged Care, visiting clients in their home and community to ensure high-quality services.
The role requires 2–3 years in a similar position, valid First Aid and Police Check, and robust communication skills to liaise with clients, families and providers.
