About the Role
As the Funding Coordinator, you will provide leadership and support in the delivery of the home's AN-ACC program, ensuring residents receive appropriate funding aligned to their care requirements. You will collaborate with clinical teams, allied health professionals, and medical practitioners to maintain high-quality documentation and assessment processes
Key Responsibilities
- Coordinate and manage AN-ACC processes across the residential home.
- Review resident care needs and identify opportunities for funding optimisation.
- Ensure clinical documentation, assessments, care plans and progress notes accurately support funding claims.
- Partner with Registered Nurses, Clinical Managers, Allied Health Professionals and Medical Officers to gather supporting evidence.
- Monitor resident changes and facilitate reassessments where appropriate.
- Deliver education, coaching and support to clinical teams on AN-ACC requirements and best-practice documentation.
- Coordinate AN-ACC meetings and support assessment visits.
- Provide guidance on predicted funding outcomes for potential admissions.
- Work collaboratively with leadership teams to drive continuous improvement and compliance.
To be successful, you will have
- Current registration with AHPRA as a Registered Nurse.
- Strong clinical assessment and documentation skills.
- Excellent communication, stakeholder engagement and coaching abilities.
- Strong organisational skills with the ability to manage competing priorities.
- Experience working as a Registered Nurse within aged care.
- Knowledge of AN-ACC funding and assessment processes.
- Experience in funding, quality, compliance or clinical coordination roles.
- Understanding of aged care legislation and funding frameworks.
