Access Telehealth is seeking an experienced nurse to assess, prioritise and coordinate care for residents in Traralgon. This on-site role is 0.7 FTE with no night or weekend shifts, working alongside GPs and Specialists to ensure timely, high-quality care.
You’ll use digital tools and clinical systems to manage referrals and care plans. You’ll bring robust clinical reasoning, risk recognition and independent decision-making, with 2+ years post-registration experience and aged care exposure.
