About the Position

The Registered Nurse (Medical) is responsible for assessing, planning, implementing and evaluating nursing care for residents presenting with medical conditions that require clinical assessment, medical intervention, treatment and ongoing monitoring. Working within Wattle Hill Care’s residential aged-care setting, the position focuses on the clinical management of residents with complex and chronic medical conditions, including monitoring their response to prescribed treatments and identifying changes in clinical status that require medical review.

The successful applicant will collaborate with general practitioners, treating medical practitioners, allied health professionals and other members of the healthcare team to coordinate evidence-based nursing interventions, medication management, clinical monitoring and continuity of treatment. The role also contributes to clinical decision-making, resident and family education, and the supervision and coordination of nursing and care staff within the employee’s professional scope of practice.

Key Responsibilities

- Conduct comprehensive clinical assessments of residents presenting with acute changes in health status, infections, metabolic disorders, degenerative conditions and other medical conditions requiring nursing intervention.

- Develop, implement and evaluate individualised nursing care plans addressing identified medical conditions, prescribed treatments, clinical risks and measurable care outcomes.

- Monitor residents’ clinical status, including relevant observations and responses to treatment, and promptly escalate deterioration or other significant clinical changes to the treating medical practitioner or appropriate emergency service.

- Administer prescribed medications and undertake nursing interventions and treatments in accordance with medical orders, qualified standards, facility protocols and the Registered Nurse’s scope of practice.

- Monitor the effectiveness and safety of prescribed treatments, identify potential complications or adverse responses, and communicate relevant findings to treating practitioners.

- Coordinate with general practitioners, medical specialists, pharmacists and allied health professionals regarding treatment plans, medication reviews, clinical follow-up and continuity of care.

- Maintain accurate and contemporaneous clinical records documenting assessments, nursing interventions, medication administration, treatment responses, clinical escalation and communication with treating practitioners.

- Provide residents and their representatives with appropriate information about medical conditions, prescribed treatments, medication requirements and recommended health-management practices.

- Contribute to the prevention and management of clinical complications, including infection-related deterioration, medication-related risks, dehydration and other risks identified through individual clinical assessments.

- Supervise and coordinate Enrolled Nurses and care staff in delivering nursing interventions and monitoring residents’ conditions, consistent with delegated responsibilities and professional accountability.

- Participate in clinical handovers, case conferences, incident reviews and quality-improvement activities to support safe, coordinated medical and nursing care.

- Comply with applicable legislation, AHPRA registration requirements, professional standards, medication-management procedures and aged-care quality and safety requirements.
