CARE MANAGER | CENTRAL WEST NSW | NOT-FOR-PROFIT AGED CARE
A community-owned, not-for-profit aged care provider in regional Central West NSW, about 3.5 hours from Sydney and roughly 1 hour from Orange, is seeking an experienced Care Manager to join their residential home.

This is a hands-on role for a Registered Nurse who loves being on the floor and takes pride in helping older people receive safe, respectful and compassionate care. The home has a genuine, home-like feel where residents are known and respected, and the provider is looking for a outstanding mentor who wants to build a long-term career in a stable, supportive team.

The Opportunity / Main Duties

- Provide day-to-day clinical leadership across shifts in a home of 80+ beds, including a secure dementia unit and independent living units.
- Be a visible presence on the floor, monitoring care and responding to changing resident needs.
- Support clinical decision-making, prioritisation and escalation.
- Coach and mentor Registered Nurses and care staff, helping less experienced RNs build confidence and sound clinical judgement.
- Oversee clinical documentation, care planning, medication practices and incident follow-up.
- Maintain high standards in wound care,



infection prevention and control, continence, dementia and palliative care, falls prevention and clinical risk management.
- Build constructive relationships with residents, families, GPs, Nurse Practitioners and visiting clinicians.
- Contribute to audits, continuous improvement, compliance and clinical governance.

What We’re Looking For

- Current AHPRA registration as a Registered Nurse, with full working rights in Australia.
- At least 5 years’ experience in Australian residential aged care. 10 or more years will be highly regarded.
- Experience in a senior clinical, care management, shift-leadership or team-leadership role. Experienced RNs ready to step up are encouraged to apply, particularly those with solid aged care experience and long tenures.
- Strong knowledge of residential aged care clinical practice, medication management, documentation, quality standards and regulatory requirements.
- Practical experience in nurse-initiated assessment and treatment, in line with policy and scope of practice.
- Confidence in identifying clinical deterioration and escalating appropriately to GPs, Nurse Practitioners, emergency services and families.
- A proven ability to coach and develop nursing and care staff, with strong communication skills and sound judgement.
- The ability to meet pre-employment screening, immunisation and aged care worker requirements.
