Multicultural Care is seeking an experienced Clinical Aged Care Assessor to join our Care Navigations Team. In this community-based role, you'll conduct comprehensive aged care assessments to help older people access services while remaining independent, managing your own caseload within a supportive team.
You'll assess clients' clinical, functional and psychosocial needs, develop person-centred support plans, maintain records, and communicate with clients, families and health specialists.
