Catholic Archdiocese of Brisbane – Moreton Bay region is seeking a Clinical Care Partner to deliver person-centred care, clinical assessments and coordinated home-based support for older Australians. Based mainly in Caboolture with travel to clients’ homes, you will work with a multidisciplinary team to ensure protected, dignified care and maximise independence.
You’ll provide guidance to Care Partners and adhere to aged care standards.
